The following are required documents for admission to Saint Mary Academy. Please return the application forms and required information to the main office of Saint Mary Academy or mail to Saint Mary Academy, 222 Central Avenue, Dover, NH 03820.
This information, along with your personal interview with the principal if you are applying for grades 1 through 8, will be required before a decision can be made with regard to admission.

Please submit the following:
- Application
- Application fee of $50.00 made payable to Saint Mary Academy
- Copy of birth certificate
- Copy of baptismal certificate (Catholics only)
For Grade 1, please also include:
- Kindergarten progress reports
- School recommendation form
- Any relevant testing
For Grades 2-8, please also include:
- Report cards
- Standardized test scores
- IEP or 504 plan (if applicable)
- "Permission to Contact Teacher" form
- Essay "Why do you want to attend Saint Mary Academy" (for grades 5-8 only)
